Andrew Dale Capps, age 28, from Lincoln, Nebraska, passed away Friday, April 17, 2015 in Phoenix, Arizona.
Survivors include his grandmother, Andrea, his mother, Pamela, his sister, Leah, his wife, Ashley, his daughters, Emma and Maisy and his son, Micah. Andrew was preceded in death by his grandfathers, Lloyd and Dale Leonard, his grandmother Norma and his father, Mark.
A memorial service will be held at 2 p.m., Thursday April 23rd at San Tan Mountain Funeral Home in Queen Creek, Arizona. Memorials in care of: San Tan Mountain Funeral Home, 21809 W. Ellsworth Rd, Queen Creek, AZ 85142 Ph. 480-888-2682
